Take an Active Role in your Pain Care

Having chronic pain can impact all aspects of your daily life. Part of chronic pain treatment at the TPS involves strategies to help manage pain day to day.

 

There are many ways that you can take an active role in managing your pain with the support of the clinical team at the TPS.

Below you will find self-management resources that you can use on your own. If you're not sure which resources are best for you, your clinician can point you in the right direction during your next clinic visit.

What does the Transitional Pain Service (TPS) do?

The TPS clinic helps patients deal with pain after surgery in the transition from hospital to home. We also work with patients who take high dose opioids and need help with medication management and opioid weaning.

 

We offer a range of services to help patients manage their pain problems according to their unique needs.

 

Our multidisciplinary team works with patients and their families to develop a pain management plan that takes advantage of one or more of the following services available at the clinic: medication management, interventional pain procedures, psychotherapy, physiotherapy, acupuncture, and patient education.

How long are patients usually followed by the TPS clinic?

Patients are usually followed for 6 weeks up to 6 months by TPS clinicians. The exact duration depends on each patient's needs and progress.
